Health Connections Mendip
Working with the Health Connections Mendip team, the successful introduction of Health Connectors in all the Mendip practices. Undertaking social prescribing and signposting, they support the clinical team with non-medical patient contact. All computers in the practices are able to access a unique Directory of Services via one key with the referrals being recorded in the patient record.
Mendip Enterprise Search and Report
A Mendip wide search facility which returns anonymised figures to support service development and patient identifiable data back to practices.
Mendip Integrated Health and Wellbeing EMIS platform
This Platform enables the management of complex patients by the Health Connections and Mendip Symphony teams with the continuous synchronisation of patient data between the Mendip Integrated Health and Wellbeing system and the practices’ own clinical system. Agreement has been made by the group to use one care plan which will allow input by multiple providers.
Pilot project trialling video consultations
Initiated by a successful Primary Care Transformation Fund bid, 4 practices are currently piloting video consultations. If the trial proves successful, it is planned to make this software available to all 11 practices by March 2018.
Practice reporting
Mendip Integrated Health and Wellbeing co-ordinates the reporting for all 11 practices to minimise the administrative workload on individual practices. 8 of the 11 practices are taking part in the Somerset Practice Quality Scheme, an alternative to QOF (Quality Outcomes Framework). The quarterly reporting requirement for this project is undertaken by Mendip Integrated Health and Wellbeing, along with participation in Quality Improvement projects and co-ordination of practice requirements for NHS GMS/PMS contracts.
Mendip Symphony
Based on the Frome Medical Practice model of enhanced primary care to prevent unplanned admissions into hospital. Multi-disciplinary team working with community services, adult social care and outside providers and unified care plans ….

All 11 practices are working to ensure uniform self-care information is available to patients on their websites.
